Comprehensive Management Plans

What is a Comprehensive Management Plan?

A Comprehensive Management Plan (CMP) is a structured, individualised care plan developed under MBS Item 139, designed for people under the age of 25 who have a confirmed diagnosis of an eligible disability. The plan is developed by a GP or paediatrician and coordinates ongoing care to support the patient's health, development, and wellbeing.

The CMP outlines the patient's current health needs, treatment goals, and the services required to support them - helping ensure all members of a patient's care team are aligned and working toward the same outcomes. Once in place, it also provides access to up to 20 Medicare-rebated allied health treatment sessions per patient's lifetime.
 
Please note, a CMP is claimable once in a patient's lifetime. If a plan has already been completed for your child or young person (including through a previous provider), a new plan cannot be created.

WHO IS ELIGIBLE FOR A COMPREHENSIVE MANAGEMENT PLAN?

Eligible Conditions/Disabilities include:​​​​​​​​​​​​​​​​​​​​

  • Sight impairment that results in vision of less than or equal to 6/18 vision or equivalent field loss in the better eye, with correction

  • Hearing impairment that results in: (i) a hearing loss of 40 decibels or greater in the better ear, across 4 frequencies; or (ii) permanent conductive hearing loss and auditory neuropathy

  • Deafblindness

  • Cerebral palsy

  • Down syndrome

  • Fragile X syndrome

  • Prader-Willi syndrome

  • Williams syndrome

  • Angelman syndrome

  • Kabuki syndrome

  • Smith-Magenis syndrome

  • CHARGE syndrome

  • Cri du Chat syndrome

  • Cornelia de Lange syndrome

  • Rett’s syndrome

  • Microcephaly, if a child has: (i) a head circumference less than the third percentile for age and sex; and (ii) a functional level at or below 2 standard deviations below the mean for age on a standard development test or an IQ score of less than 70 on a standardised test of intelligence

  • Fetal alcohol spectrum disorder (FASD)

  • Lesch-Nyhan syndrome

  • 22q deletion syndrome

  • Stuttering

  • A speech sound disorder, including: (i) an articulation disorder; and (ii) a phonological disorder; and (iii) childhood apraxia of speech (also known as dyspraxia, developmental verbal dyspraxia or speech apraxia); and (iv) dysarthria.

  • Cleft lip

  • Cleft palate

  • Cleft lip and palate

Comprehensive Management Plans for patients with a sole diagnosis of one of the following Complex Neurodevelopmental Disorders must be completed by a paediatrician or psychiatrist:

  • Autism Spectrum Disorder (ASD)

  • Attention Deficit Hyperactivity Disorder (ADHD)

  • Complex neurodevelopmental conditions with multi-domain impairment

OBTAINING A COMPREHENSIVE MANAGEMENT PLAN

A CMP can be initiated through two pathways: via a paediatrician (if the patient has one) or through their regular GP.

Option 1: Through a Paediatrician (Recommended)
If your child or young person is already under the care of a paediatrician, we recommend requesting the CMP through them in the first instance. Your paediatrician will have the most comprehensive view of the diagnosis history and treatment needs, making them best placed to complete the plan - particularly where the primary diagnosis is a complex neurodevelopmental disorder such as ASD or ADHD.

Option 2: Through Your GP (if appropriate)
If the patient does not have a paediatrician or psychiatrist, please book a standard appointment to discuss eligibility with your Bondi Doctors GP.
